Overview
WIN Care Events connects healthcare events to the organizations responsible for acting on them. Built on the WIN Platform, Care Events uses ADT notifications to initiate and coordinate, track and measure follow up work across organizational boundaries. Based on defined program criteria, Care Events can create structured work in ServiceNow, apply risk and priority rules and route transition-related tasks to the appropriate organization and care team.
This allows healthcare, community-based and social care organizations supporting the same people to coordinate around a care transition without requiring every participant to operate within the same system or process. Care coordinators see the transition, available clinical context and assigned work through the Transition of Care experience, while the broader workflow establishes accountability for what needs to happen next and helps ensure required follow up is completed so no one falls through the cracks or gets lost in the system.
Care Events does not replace the infrastructure that exchanges ADT events. Instead, it builds on those existing connections by turning an event into coordinated work across the organizations responsible for follow-up.
Who Benefits
-
Care coordinators and transitions-of-care teams responsible for ensuring appropriate follow-up after admissions, discharges and transfers.
-
Healthcare, community-based and social care organizations working together to support populations whose needs extend beyond a single care setting or organization.
-
Regional and multi-organization networks coordinating transition-related responsibilities and follow-up across participating organizations.
Key Capabilities
-
Recognize when a care transition requires action: Use qualifying HL7 v2 ADT events to identify transitions that require follow up for the people or program being supported.
-
Turn the event into orchestrated workflows: Use the care event to trigger the cases, tasks and coordinated activities required for follow-up across participating organizations.
-
Coordinate responsibility across organizations: Apply configurable criteria for risk, priority, due date, managing organization and assignment group so work reaches the appropriate participant.
-
Give coordinators visibility into the transition: Bring the patient transition, case, tasks, assigned work and available clinical context together in ServiceNow.
-
Bring relevant clinical information into the work: Retrieve available FHIR information when additional context is needed to support follow-up and coordination including diagnoses, medications, laboratory results, allergies, care team information, and discharge instructions.
-
Support reporting and escalation: Provide visibility into follow up activity and surface work that requires additional attention or escalation.
How It Fits Within WIN
WIN Care Events provides the event-to-workflow capability within the broader WIN model. When an admission, discharge or transfer requires follow up, Care Events uses that event to initiate and route work across the organizations responsible for supporting that person.
Within a Capability Package, Care Events can be configured around the participants, requirements and designed outcomes of a specific population or program.
- Subscribes to ADT (admit/discharge/transfer) events on the WIN Platform event bus
- Parses HL7 ADT messages (patient, encounter, diagnosis, facility)
- Upserts HCLS Patient and creates HCLS Encounter; creates a WIN Case linked to patient and encounter
- Risk-stratifies and routes Care Event Tasks to the right care team by risk tier, population, and managing organization
- Discharge Event Viewer for one-click access to discharge clinical context from the task
